To ensure that you are properly protected, the policy that you select must cover the destination that has the highest rating. For example if you are travelling to France, Germany and the United States you would need to select a worldwide policy including the USA, Canada and the Caribbean.
If you are travelling to Italy, Spain and India you can select a worldwide policy excluding the USA, Canada and the Caribbean.
